A Name Change can be issued to your PASSPORT IF:
You are able to submit your most recent US passport.
You can document that your name has legally changed since the passport was issued.
YOUR valid PASSPORT IS NOT HEAVILY DAMAGED OR MUTILATED.
Your most recent passport was issued when you were 16 years of age or older.

If you can not meet any of these requirements, new passport instructions apply.

1. Most recent U.S. passport.

2. One passport size photo, color, 2x2 with white background. ($15.00 for a set of two if taken in our office. Tax included.)

    • Your photographs must be: 2x2 inches in size
    • Taken within the past 6 months, showing current appearance
    • Color
    • Full face, front view with a plain white or off-white background
    • Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
    • Taken in normal street attire. Uniforms should not be worn in photographs except religious attire that is worn daily.
    • Do not wear a hat or headgear that obscures the hair or hairline.
    • No glasses may be worn in the photo.

3. If your current passport was issued within the past year: One completed Reapplication form DS5504 form. This form must be completed online. The passport office will not accept hand-written DS-5504 applications. This form must have the original signature of the applicant.

If your valid passport was issued more than one year ago: One completed DS-82 Form. This form must be completed online and printed on two pages (not back to back). The passport office will not accept hand-written DS-82 applications. This form must have the original signature of the applicant.

4. A Certified Copy of either your Marriage Certificate, Divorce Decreee, or Court Order Change of Name Decree.

The normal expedited process is taking between 4 and 6 weeks. At this time, we are doing applications by appointment. Please call us at (248) 244-8470 to schedule your appointment

Government fee is $209.53.

Our service fee is $125.00.

We do offer emergency processing between 5 and 12 business days. If you need this processing time, please contact us for requirements, fees, and to set up an appointment.
